Scope

Has frequent contact with clients, family, fellow nurses, and various members of the medical community. No positions report to the Home Care RN.

Position Summary

Administer skilled nursing care to one specific client who requires professional nursing services. These services are performed in accordance with the physician’s orders and under the direction and supervision of the Home Care Manager. Perform duties in any nursing capacity as needed over a 12 hour shift. This position is a 7am to 7pm shifts. 3 shifts a week and every other weekend and every other Holiday.

Requirements

  • Required to have the ability to read, write, and speak English well enough to communicate with staff, clients, and visitors. Must complete and pass a state required background study.
  • Graduate of an approved school of professional nursing. Licensed registered nurse in the state of Minnesota. Able to communicate verbally and in writing to the extent required by the position. Able to physically perform the duties required by the position. Must have a car with required insurance coverage and a state driver’s license to travel to client’s residence. Must have adequate computer skills.

Responsibilities

  • Provide Skilled Nursing Care
    • Nursing services, treatments, and preventative procedures requiring substantial specialized skill and ordered by the physician. Initiate preventative and rehabilitative nursing procedures as appropriate for the client’s care and safety. Observe signs and symptoms and report to the physician and Home Care Manager reactions to treatments, including drugs, and changes in the client’s physical and emotional condition. Teach, supervise, and counsel the client and caregivers regarding the nursing care needs and other related problems of the client at home. Communicate with Home Care Manager any changes in the client’s condition.
  • Evaluation and Documentation of Client Care
    • Chart services rendered to the client, changes observed in the client’s condition and/or family and home situation, inform the Home Care Manager of potential revisions in the nursing care plan as needed, report client care and progress, prepare paperwork required for continuing care. Evaluate the effectiveness of nursing service provided to the individual and family. Submit clinical notes, progress notes, and other clinical record forms outlining the services rendered in a timely manner. Participate in case conferences. Discuss with Home Care Manager problems concerning the clients and how they may best be handled. Discuss with the Home Care Manager the need for involvement of other members of the health care team such as physical therapist, speech therapist, occupational therapist, social worker, etc. Cooperate with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan.
  • Continuous Improvement
    • Participate in staff development meetings. Participate in the educational experiences for student nurses. Continually strive to improve nursing care by attending in-service education, through formal education, attendance at workshops, conferences, active participation in professional and related organizations and individual research and reading. Participate in the planning, operation, and evaluation of the nursing service. Participate in the development and periodic revision of the physician’s Plan of Care and processes change orders as needed. Maintain an on-going knowledge of current drug therapy.
